Generous Spireites fans helped to raise £800, which was recently handed over to the club's charity partner, Ashgate Hospicecare.
The sum was partly made up of cash donated by Spire Lotto prize-winners who chose to make a donation rather than collect their prize.
The total also included money paid by fans who attended a managerial Q&A event earlier in the season, together with a contrbution from the auction of a Coronation Street script, which was purchased by club sponsor Karen Child at the 'Corrie Night'.
Alison Ward-Foster, Ashgate Hospicecare's partnerships manager, received the cheque from Karen Child at half time in the game against FC Halifax Town. She said: "Thank you so much to Karen Child and Chesterfield Football Club! The generous donation of £800 could pay for 11 patients to receive a visit from our hospice at home team, enabling them to stay in their own home surrounded by those they love.
"All Ashgate Hospicecare services are provided free of charge and in order for us to maintain the specialist palliative care we provide, we rely on the support of our community, which is why this donation is so important. Thank you for helping our nurses to make a real difference to the lives of our patients and their families."
